Insurance Carrier: Stillwater Mineing Co. / EBMS
State: Nebraska

Consumer Complaint:

EBMS and Stillwater Mining company are refusing artificial disc replacement in the cervical spine because it is not medically necessary and experimental and investigational. I have documentation to prove otherwise.

If you have a self-insured plan it is NOT an insurance product issued by an insurance company and we cannot help with suggestions, other than to say see a lawyer.
