Do I need to file a claim?


Yes. If you have any written material from your employer or insurance company, check that first for any instructions. Otherwise, begin by contacting your company benefits person for a group plan or your insurance company for a personal plan. They will send you a claim form if you do not already have one and tell you how to proceed.
